New Patient Registration
If you wish to register with the practice you can email the registration forms to us or print them off and hand in to the surgery along with photographic identification and proof of address if also signing up to Patient Online Services.

Fill out the application form and email it, along with the new patient questionnaire forms, to us at tay.strathmoreprescriptions@nhs.scot
Alternatively you can print it off along with the new patient questionnaire forms, complete them and take them along to the surgery.
There is one new patient questionnaire for adults and one for children aged under 16 years.

Temporary Registration
If you are staying within our practice area for more than 24 hours but not more that 3 months you can register with the practice on a temporary basis. If you stay longer than 3 months you will need to register on a permanent basis. We cannot register you as a temporary patient again after this 3 month period has ended. Please telephone reception for more details.
